Meropenem is commonly prescribed in critically ill children for targeted therapy in severe and multidrug-resistant bacterial infections. Meropenem, as a carbapenem, has a time-dependent bactericidal effect. Optimal dosing is evaluated based on the ability to maintain meropenem concentrations in plasma above the minimum inhibitory concentration (MIC) of the infecting pathogen throughout the dosing interval (fT > MIC) (1, 2). However, the optimal clinical pharmacokinetic/pharmacodynamic (PK/PD) target remains uncertain as it varies from 50% fT > MIC to 100% fT > 4× MIC. The latter would be desirable for critically ill patients (3, 4)
